Altitude Sickness

Altitude sickness, or acute mountain sickness (AMS), occurs at high elevations when the body doesn’t adjust to reduced oxygen levels. Severe cases can lead to fluid buildup in the lungs or brain, potentially fatal if untreated. Acetazolamide (Diamox) can be used to help prevent and reduce symptoms of altitude sickness.

Altitude sickness, also referred to as acute mountain sickness (AMS), is a condition that affects individuals who ascend to high altitudes too rapidly, typically above 2,500 metres. The condition arises due to reduced air pressure and lower oxygen levels, which the body has not yet adapted to. When a person travels to a higher elevation without allowing sufficient time for acclimatisation, the body struggles to cope with the decreased availability of oxygen. While altitude sickness is usually mild and temporary, it can occasionally develop into more serious forms such as high-altitude pulmonary oedema (HAPE) or high-altitude cerebral oedema (HACE), which are medical emergencies requiring immediate attention.

Symptoms of altitude sickness often start within a few hours of reaching a higher altitude. The most common signs include headache, nausea, vomiting, dizziness, tiredness, loss of appetite, and difficulty sleeping. Many individuals also experience shortness of breath, especially during physical exertion, and may feel generally unwell or irritable. These symptoms can range from mild to severe and may worsen if the person continues to ascend without proper acclimatisation. In some cases, confusion, chest tightness, and coordination problems can occur, indicating a progression to more severe altitude-related illnesses. Recognising these symptoms early is crucial for effective management.

The primary treatment for altitude sickness is to stop ascending and allow the body to acclimatise. Rest and hydration are essential, and it is advised to avoid alcohol and strenuous activity. If symptoms persist or worsen, descending to a lower altitude is often necessary. Mild cases can be managed with over-the-counter painkillers like paracetamol or ibuprofen for headaches, and anti-nausea medication for digestive symptoms. For prevention or more persistent cases, a doctor may prescribe acetazolamide (Diamox), which helps the body acclimatise more quickly by encouraging deeper and more frequent breathing. This medication is generally suitable for adults and children over the age of 12, but should only be used under medical advice. In more severe cases, dexamethasone may be given as an anti-inflammatory treatment, especially if descent is not immediately possible.

Although generally effective, medications used to treat altitude sickness can have side effects. Acetazolamide may cause tingling sensations in the hands and feet, frequent urination, altered taste, and, occasionally, gastrointestinal discomfort. Some people report increased fatigue or mild allergic reactions. Dexamethasone, being a corticosteroid, can lead to temporary mood changes, increased appetite, insomnia, and elevated blood sugar levels. When used for short periods and under medical supervision, these side effects are typically manageable and outweighed by the benefits of the treatment. However, prolonged or unsupervised use of these medications is not recommended due to the risk of more serious adverse effects. Always consult a healthcare professional before beginning any altitude sickness medication, especially for children or individuals with pre-existing medical conditions.
